EAP - Enfermedad arterial periférica es unaaplicación dirigida al colectivo médico, desarrollada conjuntamentepor Ferrer Internacional, S.A. y Otsuka Pharmaceutica, S.A., paradetectar y diagnosticar de forma rápida y fiable la EAP mediante elCUESTIONARIO DE CLAUDICACIÓN DE EDINBURGH y EL ÍNDICETOBILLO/BRAZO.La EAP es la afectación aterosclerótica de las arteriasperiféricas: carótidas y vertebrales extracraneales, de lasextremidades superiores, de las extremidades inferiores, renales ymesentéricas (excluyendo la aorta, las coronarias y lascerebrales). La arteriosclerosis se desarrolla lentamente,reduciendo la irrigación tisular subsidiaria.La prevalencia de esta patología aumenta con la edad y será una delas dolencias más frecuentes por el progresivo envejecimientodemográfico.¡1 de cada 5 personas mayores de 65 años padece EAP!Muchos pacientes permanecen asintomáticos, puesto que laisquemia solo se manifiesta cuando hay demanda funcional, ygeneralmente estos enfermos son sedentarios o casi absolutamenteinactivos. En ocasiones algunos síntomas pueden agravarse, y puedentener lugar episodios agudos, a menudo asociados con trombosis,embolias u oclusión de una arteria principal.El síntoma principal es la claudicación intermitente (CI), osíndrome del escaparate: un dolor en la zona de la pantorrilla queaparece al caminar y desaparece rápidamente al descansar.¡Pero sólo 1 de cada 10 pacientes con EAP presenta estesíntoma!Hay que realizar un despistaje activo y sistemático de la EAP enlos pacientes ancianos (›65 años), fumadores, diabéticos,hiperlipémicos, hipertensos, con otras enfermedadescardiovasculares (insuficiencia renal, coronariopatía, enfermedadcerebrovascular) o con clínica sugestiva (CI, lesiones o atrofiasdistales). Un alto porcentaje de los pacientes con EAP presentantambién afectación coronaria o cerebrovascular, y tienen un elevadoriesgo de sufrir acontecimientos cardiovasculares graves.- El CUESTIONARIO DE CLAUDICACIÓN DE EDINBURGH permite detectary diagnosticar la claudicación intermitente mediante 6 preguntasrápidas y sencillas a su paciente.- El ÍNDICE TOBILLO/BRAZO (ITB) confirma el diagnóstico de EAP(‹0,9) y determina su gravedad. El ITB se calcula dividiendo lapresión arterial sistólica (PAS) de cada tobillo entre el valor dela PAS en el brazo. Colocaremos manguitos de presión de 8-12 cm enambos brazos y en ambos tobillos al paciente en decúbito supino.Tras 10 minutos de reposo, mediante una sonda Doppler de 5-10 MHz,sin presionar la arteria y formando un ángulo con ella de 45º,localizaremos el flujo arterial y mediremos la PAS en las arteriashumerales, pedias y tibiales posteriores. Tomaremos el valor mayorde los brazos y el mayor de cada tobillo (pedia o tibialposterior).
